The controller is Smile Centrum s.r.o., Company ID No.: 03496902, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, File No. 102854, with its registered office at Trmická 835/4, Prosek, 190 00 Prague, which, as a legal entity, processes your personal data for the purposes specified below.

• online appointment booking (based on completion of the appointment form),
• provision of dental services (based on completion of the registration form),

When you book an appointment, we collect the following personal data:

• a description of the requested procedure or problem,
• telephone contact details,
• e-mail address,
• the date, time and location of the requested treatment.

Personal data for direct marketing

If you have given our company consent to process your personal data for direct marketing purposes, these data will be used to send commercial and non-commercial communications to your e-mail address.

In particular, we process the following personal data:

• first name and surname,
• e-mail address,
• date of birth.

With whom do we share your personal data?

Your personal data are processed through our software system, which can be accessed only by authorised employees of the controller.

Personal data processed for direct marketing purposes are not shared with any third parties and are used exclusively for the needs of our company.

Our company has also entered into a joint personal data processing agreement for an indefinite period in order to improve the quality of the services provided. Under this agreement, your personal data may also be shared with ProSmile Clinic s.r.o. and Family Dental Care s.r.o.

Our company is also designated as the contact point for data subjects. This does not affect your right to contact the authorised data protection officer.

Transfer of personal data abroad

Your personal data will not be transferred outside the Czech Republic or the European Union.

How long are personal data retained?

Personal data provided when booking an appointment online are retained for the period strictly necessary to fulfil the purpose of their processing, in particular to ensure the provision of the requested dental services.

Personal data are retained on the basis of the applicable legal grounds for their processing. Where processing is based on consent, the data are retained until that consent is withdrawn. After consent has been withdrawn, the personal data are erased or anonymised in accordance with applicable legislation.

Personal data processed for direct marketing purposes are retained until you withdraw your consent.

Access to your personal data and the right to rectification

You have the right to request access to your personal data at any time. You also have the right to obtain confirmation as to whether your personal data are being processed and, if so, to receive a copy of them.

If your personal data change, you may ask us to update or complete them. Our company will rectify or complete the personal data without undue delay after receiving your request.

Erasure and restriction of the processing of your personal data

Personal data processed on the basis of consent for marketing purposes will be erased without undue delay after receipt of a request for erasure or immediately after consent to the processing of personal data has been withdrawn.

If you no longer wish your personal data to be processed by our company and would like them to be processed by another controller, you may ask us to transfer them. Objection to the processing of personal data

Automated individual decision-making, including profiling

Your personal data will not be used for automated individual decision-making or profiling.

How to withdraw your consent to the processing of personal data

You may withdraw your consent to the processing of personal data at any time and in any form, in person, by e-mail at info@smilecentrum.mom, or by post at: Smile Centrum s.r.o., Trmická 835/4, Prague 9, 190 00.

Consent may also be withdrawn partially, i.e. only in relation to certain personal data, or in full. After consent has been withdrawn, the personal data will no longer be processed on that basis and will be erased or anonymised in accordance with applicable legislation.

The withdrawal of consent applies only to data processed on the basis of consent and does not affect the processing of personal data carried out on the basis of law or in connection with the provision of healthcare services.

What should you do if your personal data protection rights are breached?

If you believe that your rights have been infringed in the processing of your personal data, in particular that the personal data are not being processed in accordance with the stated purpose or within the defined scope, you have the right to contact the Office for Personal Data Protection, with its registered office at Pplk. Smile Centrum s.r.o. uses cookies to analyse website traffic through Google Analytics. Their purpose is statistical analysis and improvement of the website’s functionality.
